Fidelity Tools

To complete the surveys your district must have established a PBIS Assessment Local Coordinator. If your district does not have a PBIS Assessment Local Coordinator, please complete this Local Coordinator Information Form and email it to Melissa Marks to become your district's PBIS Assessment Local Coordinator. If you are unsure if your district has a PBIS Assessment Local Coordinator, please read the following PBIS Assessment Procedure guide.
Tier 1/Universal Fidelity Tools
Self-Assessment Survey

Conducted initially and then annually - preferably in the Fall each year
- Completed by all staff that have contact with students
- Determines the status of PBIS in a school as perceived by all staff members
- Guides the development of an action plan for improving PBIS implementation
- Schools scoring 80% or higher on the Implementation Average are implementing PBIS at the Universal/Tier 1 level with fidelity
Team Implementation Checklist

Completed in the Fall and Winter each year until the Spring Benchmarks of Quality score is at or above 70%. Teams may choose to take the TIC more often to assist them with Action Planning their PBIS implementation.
- PBIS team reaches consensus on each item prior to entering result in PBIS Assessment
- Guides the development of an action plan for improving PBIS implementation
- Schools scoring 80% or higher are implementing PBIS at the Universal/Tier 1 level with fidelity. Instructions for calculating your total score can be found here.
Benchmarks of Quality

Completed annually in the Spring
- Completed by Internal Coach and each member of PBIS team
- Assists team in finding areas of strength and areas in need of improvement in PBIS implementation
- Schools scoring 70% or higher are implementing PBIS at the Universal/Tier 1 level with fidelity

Benchmarks for Advanced Tiers
- Completed annually in the Spring
- Completed by team as a group or by each member independently with discussion/consensus following
- Assists team in finding areas of strength and areas in need of improvement in targeted and intensive PBIS systems and interventions
- No fidelity score cut-off has been established yet for this tool
