School-wide Information System (SWIS) was created by the OSEP Technical Assistance Center on PBIS as a web-based data collection tool to support the data-based decision making of PBIS teams around discipline data. Collecting and aggregating discipline data is vital to the PBIS process. Schools that choose not to use SWIS must have a method for collecting and aggregating their discipline data. If another method is not available, please feel free to use this Outcome Data Collection Tool. This tool was created in Microsoft Excel by Nicole Beier. It can be used to enter Office Discipline Referrals. It will automatically create four of the "Big 5" PBIS graphs (Average Referrals per Day per Month, Referrals by Problem Behavior, Referrals by Time, and Referrals by Location). Creation of the fifth graph (Referrals by Student) takes a bit of Excel knowledge. If you decide to use this tool, please feel free to contact Melissa Marks if you have any questions or comments. If you already enter Office Discipline Referral Data into a Student Management System, but that system does not create graphs. You may find this Aggregate Outcome Data Collection Tool useful.
